Overview of HealthPathways in the ACT

What is HealthPathways?

HealthPathways evolved in the ACT through collaboration between Capital Health Network (ACT PHN), ACT Health, COORDINARE (SENSW PHN) and Southern NSW Local Health District. It is a web based portal designed to support general practitioners in planning patient care through primary and secondary health care systems within the local region.  HealthPathways will help health practitioners manage and refer patients to the right care, at the right place, at the right time.  


When can I start using HealthPathways?

The ACT & Southern NSW HealthPathways portal went live on 13th April 2015. 

Over 185 clinical, referral and resource pathways have now been localised for the ACT & Southern NSW region.
